New Patient Registration
To register with the practice as an NHS patient you are required to come to the surgery and complete the appropriate forms with proof of identification and local residency. The practice reserves the right to refuse applications.
Please bring the following forms with you to surgery:

To register you will need:
Mandatory NHS number - if known
- Photo ID - Passport or Id card proving country of origin
- Utility Bill or Bank statement dated within the last 3 months with your own details on. You maybe asked for the following : Home office work permit document - if applicable
- Work/Resident permit visa - if applicable
- Student visa plus letter from college to show course and proof of attendance
For children registering in the UK from abroad, please provide a record of immunisations they have had.
Patients joining the practice are registered with the Herschel Medical Centre, but one clinician will have overall responsibility for your care. However, you are able to see any member of the clinical team subject to availability. Often your care will be from the practice nurses for on-going conditions such as diabetes, asthma dressings or family planning, and the healthcare assistants for simple tests and procedures such as blood pressure and urine checks.
