Lloyds Chemists in Slough High Street has now closed. Most patients who have had repeat prescriptions sent to that pharmacy have already been notified by Lloyds and have changed their nominated pharmacy. If you are a patient who hasn't picked up their January's prescription from Lloyds, Slough High Street, please contact the surgery to ensure your prescription request can be dealt with.

We have made a slight change to our prescription form. The form is very similar to the existing form and self explanatory but you will need to delete any bookmarks you have to the existing form as it will no longer be available.

Repeat Prescriptions

Patients on regular medication are given a repeat prescription form which is attached to the right hand side of the original prescription. This indicates all medications being used on a regular basis.

To re-order any repeat medications we do ask that this side of the prescription is detached and handed to the reception or posted to the surgery. (If you require the prescription to be returned by post you must supply a stamped addressed envelope.)

To ensure accuracy of prescription issues telephone requests for repeat prescriptions are not accepted.

Prescriptions can be collected from the surgery allowing 2 full working days for completion.

Alternatively for convenience and after prior arrangement, prescriptions may also be collected by your local pharmacy on your behalf after 2 full working days.

Reviewing Your Repeat Medication

The clinicians review your repeat prescription medication regularly. If they need to see you for a review, a reminder will be attached to the prescription for your attention. If you think your medication or the level it is prescribed at should change, please contact the surgery for an appointment.

Pharmacy Collection Service

The surgery works closely with local pharmacies who will collect your prescription on your behalf from the surgery and prepare for your collection directly at the Pharmacy. Should you wish to engage in this service, please contact the prescription department who will arrange for you.
